The fire occurred at a property in the town of Tullamore, Co Offaly, on the night of 15 December 2025. The victims were identified locally as four-year-old Liam O'Connor and his grand-aunt, 72-year-old Mary O'Connor.

Gardaí arrested three people, two men and one woman, on 30 April 2026 as part of the investigation. They were questioned at Tullamore Garda Station and released without charge on 1 May 2026. A file is being prepared for the Director of Public Prosecutions.
